Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment.
After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.
We will inform you of all costs prior to treatment. Our office accepts payments via Visa, MasterCard, Discover Card, cash and checks. We also offer payment plans through CareCredit and Capital One dental financing companies, which offer many different options including “90 days no interest” among others.
We pledge to thoroughly review our payment options to accommodate your financial situation.
If special arrangements are needed, please talk to our office manager before receiving service.
As a courtesy, we will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, please remember you are personally responsible for your account. We encourage you to contact us if your policy has not paid within 30 days.
